Level I Training Conference is October 2-3, 2009.

Level II Training Conference is October 4, 2009.

Refund Policy: No Refunds will be issued if cancellations are received one month, or less, before the event.

The Fountain Hills Community Center is a beautifully appointed conference center with wonderful views of the World Famous Fountain.

Fees for the conference are located on the Conference Registration Forms available under Level I, and Level II.  Membership prices are located under each Membership Category under "Membership".

For example a Physician Membership is $850.  Level I for a Physician is $750 & Level II for a Physician is $500.  Clicking on the forms to look at the pricing will not automatically register you for the event.

If you have previously attended Level I and/or Level II Part I and would like to audit these conferences please register under Audit on the Conference Registration Forms for Level I and/or Level II.  The cost is $225.00 to audit Level I and $175 to audit Level II.

Product and Service Exhibitors will be situated in the spacious and light- filled Exhibitors area, located just outside the doors of the classroom and lunch room.
